In-home care: Meeting patients where they are

Today, nearly half of the 13 million people who are dually eligible for Medicare and Medicaid are not in good health. The home is becoming an optimal setting for delivering value-based care, especially for patients who are most in need and have limited access to care.

Antonio’s story

Managing multiple chronic illnesses can be overwhelming, especially for older patients trying to keep up with medications, doctor’s appointments and daily health routines. Within this decade, 20% of Americans will be age 65 or older. Many, like Antonio, cope with complex health needs and often prefer to stay in their own homes, where they feel most at ease.

Patient-centered, quality care

Antonio was assigned to an individualized care team, including a dedicated care navigator, Daphne. Daphne checked in with Antonio regularly, ensuring he attended appointments, received vaccinations and accessed necessary community health resources. Daphne worked closely with Antonio’s nurse case manager, who visited Antonio each month and oversaw his progress. 

Fast forward two and a half years, and Antonio’s transformation is remarkable. He is now 100% compliant with his medications, follows a healthy diet and monitors his blood sugar regularly. His latest A1C reading was an impressive 5.4, a major improvement from where he was before. 

“They treat you like family,” Antonio says. “If you don’t understand something, they won’t stop until you do. You can’t get any better than that.”
