Value-based care at home
Thousands of Optum clinicians are bridging the home and clinic, bringing together nurse practitioners, social workers, pharmacists and other experts to coordinate care for the growing number of patients who require more individualized care. Many have disabilities, struggle with mental health issues, lack transportation and live in remote areas. On average, these patients are managing nine chronic conditions and multiple medications.
Optum care teams serve as an extension of the patient’s primary care physician, building relationships and earning trust during in-home visits. This integrated approach leads to better outcomes, including a 12% reduction in hospitalizations.
